Ontario Ombudsman Paul Dubé’s latest report, released on 25 November 2025, reveals how the province’s systemic failure to help people with complex developmental disabilities and high needs has left too many to suffer for unnecessarily long periods in hospital.

The Office of the Ombudsman Punjab, took decisive action against the long-standing delays in payments owed to multiple stationery supplier companies of Punjab. The companies had approached the Ombudsman Office with complaints regarding delayed dues from various provincial departments, citing procedural hurdles and administrative bottlenecks.

The Office of the Provincial Ombudsman Sindh and Transparency International Pakistan have signed a Memorandum of Understanding on 11th November, 2025 to strengthen transparency, promote good governance, and ensure better, free, and speedy justice for citizens across Sindh. The signing ceremony, held at the POS Headquarters, was led by Provincial Ombudsman Sindh Mr. Muhammad Sohail Rajput and Chairman TI-Pakistan Justice (R) Zia Pervez, and attended by senior officials from both institutions.

The Office of the Ombudsman Punjab conducted a virtual training session in collaboration with the Federal Ombudsman Institution of Belgium on 5th November 2025, aimed at facilitating its officers and staff with international best practices, complaint-handling methods, and operational procedures adopted in Belgium.

On 17 November 2025, the NSW Ombudsman tabled the Reviewable Child Deaths in NSW 2022 and 2023 Biennial Report to Parliament. This report concerns the reviewable deaths of children in NSW during 2022 and 2023, and the institution's work and activities in relation to reviewable deaths since its last such report in November 2023. 

An investigation by the Commonwealth Ombudsman, released in June 2025, found the Government agency, Services Australia, is not doing enough to identify and respond to financial abuse through the Child Support program. This financial abuse is commonly referred to as 'the weaponisation of Child Support'.
