Australia | NSW Ombudsman tables Annual Report 2024–25 in milestone 50th anniversary year

The NSW Ombudsman has tabled its Annual Report 2024–25 and the NSW Child Death Review Team (CDRT) Annual Report 2024–25 in the Parliament of NSW today.

The Ombudsman’s Annual Report illustrates the important role played by the independent integrity agency, outlining operations undertaken during the 2024–25 financial year towards pursuing fairness for the people of NSW.

In 2025, the Ombudsman celebrated its 50-year anniversary milestone, and the Report pays tribute to the achievements and progress made by all those who have contributed to the agency over the past five decades. This follows the release of the ‘Fifty years of the NSW Ombudsman’ historical report which was also tabled in Parliament in July 2025.

NSW Ombudsman, Paul Miller, spoke proudly of the agency’s impact spanning 50 years of operation.

“I am honoured to hold the role during such a significant milestone as we reflect on the enduring legacy of our work and look ahead to the continuing importance and impact of our functions into the future,” said Mr Miller.

In 2024–25, the Ombudsman saw a 22% increase in contacts received (36,167) and a 15% rise in actionable complaints received (16,960). These included complaints about state government, local government, privately managed correctional centres, public universities, NSW Government-funded community service providers, and other public authorities.

The Ombudsman took further investigatory action on 3,796 actionable complaints during the reporting period – an increase of 11% from the previous financial year.

“Contacts with our office continue to grow year-on-year,” Mr Miller continued. “With an ongoing transition to cloud technologies, a new online AI assistant and a case management system upgrade, we are increasingly focused on improving the efficiency, security and customer experience for those engaging with our office.”

The NSW Ombudsman tabled 11 reports in Parliament during the year, including 7 special reports that addressed issues across the breadth of the Ombudsman’s jurisdiction. These included our first review report on the Mandatory Disease Testing Act 2021 and OCHRE 2024: Current status and future direction, examining the Government’s flagship Aboriginal affairs program.

The Ombudsman is also the convenor of the State’s Child Death Review Team (CDRT) and separately tabled in Parliament today the CDRT’s Annual Report 2024–25.

The CDRT is responsible for registering, classifying, analysing, and reporting on data and trends relating to all deaths of children aged 0-17 years in NSW. It also conducts and commissions research aimed at preventing or reducing the risk of child deaths in NSW.

This Report outlines the work of the CDRT over the last year, detailing the extent to which previous CDRT’s recommendations to prevent or reduce the likelihood of child deaths have been accepted by agencies, and the progress being made to implement recommendations.

The reports can be downloaded from the institution's website.
