The 2023-24 Annual Report of the Office of the Correctional Investigator (OCI) was tabled in Parliament on October 29, 2024. The Office’s latest Annual Report breaks new ground in two national-level investigations examining conditions of confinement in male stand-alone maximum-security penitentiaries and on the experience of persons serving a life (or indeterminate) sentence in Canada, a sentence that has no end date.

Ombudsman Ontario kicks off 50th anniversary year with Good Governance Awards
On 28 February 2025, the Ontario Ombudsman Paul Dubé announced a new award for public sector bodies that have demonstrated exemplary administrative practices – the Ombudsman’s Award for Good Governance – as part of his office’s 50th anniversary activities. Awards will be given to selected organizations in three areas of the Ombudsman’s mandate – provincial government bodies, broader public sector bodies (e.g., school boards, universities, children’s aid societies), and municipalities, he announced.

Own Initiative Investigation into possible systemic maladministration within the Corradino Correctional Facility
In 2021, as a result of persistent media reports, alleging violations of basic human rights and non-observance of prison regulations within the Corradino Correctional Facility, and after NGO “Moviment Graffitti” wrote to the Ombudsman highlighting these concerns, the Ombudsman Mr Anthony Mifsud decided to conduct this investigation in accordance with sub-article (2) of Article 13 of the Ombudsman Act (Cap. 385).

The Chief Ombudsman has referred record keeping issues identified during his investigation into an official information complaint against Associate Health Minister Casey Costello to the Chief Archivist. Peter Boshier began an investigation after he received a complaint about the Associate Minister’s refusal to release information on ‘who’ wrote or compiled notes she used to formulate the government’s tobacco policy.

UN Resolution underlines important role of Ombudsman in achieving the SDGs
On 17 December 2024, the UN General Assembly adopted the Resolution on The role of Ombudsman and mediator institutions in the promotion and protection of human rights, good governance and the rule of law. Toronto Ombudsman finds major gaps with police’s registry for vulnerable persons
On 5 September 2024, Toronto's Ombudsman released his report, An Investigation into the Toronto Police Service’s Communications About its Vulnerable Persons Registry. The TPS launched the registry in December of 2019 to allow the public to file personalized de-escalation strategies to help police officers support individuals with emotional, psychological, medical, or other physical conditions. Four years later, the Ombudsman’s investigation found that only 305 people currently have their information entered into the registry.
