The Iowa Department of Human Services (DHS) failed to closely monitor a child care center after finding violations associated with children engaging in sexual behavior, the State Ombudsman concludes in a report released today.

Ontario Ombudsman André Marin today called on the Ministry of Community Safety and Correctional Services to crack the pervasive “code of silence” under which some guards in the province’s correctional institutions use excessive force against inmates and conspire to cover it up.

Ombudsman’s findings March 2013
The Scottish Public Services Ombudsman (SPSO) laid four investigation reports before the Scottish Parliament: three about health boards and one about a local authority. The Ombudsman also laid a report on 72 decisions about all of the sectors under its remit.

Passing away of Mrs Grace Phiri
On 18 June 2013 Mrs Grace Phiri, Investigations Officer at the Zambian Commission for Investigation for 16 years, passed away in Paris on her way to the AOMA Exco Meeting in Burkina Faso. She was travelling with the Investigator General of Zambia, Mrs. Caroline Sokoni.

NSW Ombudsman’s report on child deaths
NSW Ombudsman tabled a biennial report to Parliament concerning his review of the deaths of 77 children in 2010 and 2011. Under the Community Services Act 1993, the Ombudsman is required to review the deaths of children that occur as a result of abuse or neglect.
