2025-12-18New Ombudsman Toronto report into the City’s handling of a resident’s social assistance overpayments
The City of Toronto’s Ombudsman Kwame Addo says the City’s Employment and Social Services division made multiple errors in how it responded to a Toronto mother who’d received overpayments in social assistance. Addo says City staff did not give the client a clear explanation of what she owed, neglected information that supported her case, didn’t speak to her to understand the facts and kept her waiting for hours when she tried to speak to a supervisor.

2025-12-18Hospital treatment delay led to avoidable death of man with disabilities
A 45-year-old man with disabilities was referred to hospital for intravenous antibiotics but had to wait 34 hours for the medication. By the time he received the antibiotics it was too late and he died from sepsis. The Parliamentary and Health Service Ombudsman found that his death was avoidable.

2017-06-29Last call for IOI regional subsidies 2017/18
In its efforts to further increase the regional impact of the IOI, the members of the IOI Board of Directors decided at their recent Board meeting in Vienna to continue the regional subsidy programme for the membership year 2017/2018. Institutional members wishing to apply for a regional subsidy can submit project proposal until 1 September 2017.

2024-08-01Près de 1200 signalements de lanceurs d’alerte dans le secteur privé en 2023
En 2023, première année d'application de la nouvelle loi sur les lanceurs d’alerte, les autorités compétentes ont traité 1010 signalements, qui ont donné lieu à 840 enquêtes. A ce chiffre, il faut ajouter les 209 signalements du secteur privé reçus par le Médiateur fédéral, qui en a transmis 22 aux autorités compétentes pour suivi.
