The NSW Child Death Review Team’s Annual Report for 2012 was tabled in Parliament today by the Team’s Convenor, the NSW Ombudsman. The report examines the deaths of 493 children registered in NSW in 2012. ‘The rate of child deaths is the lowest since the Team began in 1996,’ said the Ombudsman, Bruce Barbour.

Canada's Veterans Ombudsman, released his report on the New Veterans Charter accompanied by an Actuarial Analysis. The report puts forward evidenced-based recommendations that address shortcomings in three New Veterans Charter program areas: financial, vocational rehabilitation and assistance, and family support. This is the first time that recommendations for improvements have been supported by an actuarial analysis that pinpoint exactly where the current suite of New Veterans Charter benefits are failing some Veterans today.

The content of the letters written by prisoners to the authorities cannot be checked. However, such a situation occurred in Balassagyarmat Prison. Furthermore, in one case a disciplinary procedure was launched because of the tone of a letter. The Commissioner for Fundamental Rights has turned to the Director General of the Hungarian Prison Service to prevent similar incidents in the future.
