Allegations “not substantiated” in matter referred to Victorian Ombudsman by parliamentary committee
In November 2016 the case of the transfer of young people to a hastily configured youth justice and remand centre inside a maximum-security adult prison, became subject to Supreme Court proceedings. Later, the case was referred to the ombudsman by the Victorian Parliament’s Legal and Social Issues Committee. Victorian Ombudsman Deborah Glass has recommended no further action be taken after investigating the allegations.

Manitoba Ombudsman releases a report under PHIA about a privacy breach related to MRI services
Manitoba Ombudsman has released an investigation report under the Personal Health Information Act (PHIA) related to the unauthorized disclosure of personal health information of 91 patients who received magnetic resonance imaging (MRI) scans within the Winnipeg Regional Health Authority (WRHA) between 2008 and 2016. The patients’ health information was disclosed in violation of PHIA to several media organizations.

A former Council employee at the Mildura Cemetery Trust in Victoria, Australia, misused his position for personal benefit and made careless mistakes that distressed grieving families, Victorian Ombudsman Deborah Glass has found. Her investigation looked into allegations about the employee, who worked at Mildura’s Nichols Point and Murray Pines cemeteries from 1993 to August 2018.
